Description
We’re partnering with a digital health company that supports patients at risk for, or living with, chronic kidney disease (CKD), and we’re looking for Registered Nurse (RN) Care Managers to join their team. This is a remote, full-time opportunity for nurses passionate about coaching, patient education, and supporting behavior change in chronic care.
What You'll Do:
- Connect with patients via phone, video, and messaging to support their health goals.
- Use motivational coaching to guide behavior change and assess health needs.
- Develop individualized care plans (CKD experience preferred).
- Coordinate care and collaborate with providers as needed.
Requirements
What We're Looking For:
- Licensure: RN license in good standing; an additional multi-state compact license is preferred.
- Experience: 4+ years of nursing experience.
- Additional Training: CDE or CDCES, and/or training in health coaching or motivational interviewing strongly preferred.
- Bilingual English/Spanish required.
- Location: Based in the Pacific, Alaska, Arizona, or Mountain time zone required.
- Travel: Ability and willingness to travel 1-3 times per year for corporate gatherings, conferences, or health fairs, including overnight stays.
- Workspace: Quiet, secure location with reliable high-speed internet (U.S.-based only).
- Schedule: Full-time: Monday–Friday with the following shifts: 8 am - 5 pm PST (2 days) and 10 am -7 pm PST (3 days).
Details:
- Compensation: $80,000 - $100,000 annual salary
- Benefits: Health, Dental, Vision, PTO, and 401K
- Start Date: Flexible - ASAP
